but I think the standard practice is to get the blood tested by the hospital lab (irrespective of where the blood is coming from or if it has been tested or not) before handing it over to the surgical team.

Blood products are always issued by a central lab. And even when they arrive at their destinations, they are re-screened ans matched with patient blood
There is always a margin of error anywhere
