The doctor didn't provide any report for me and I don't have the latest receipt again. The one I have is the one I was given when I went to the hospital the first time. After a week when I was not healed, then I went back to the hospital and I used 13000 to treat myself.
Below is the card I was given and the first payment I made the first time I went to the hospital.
