Now to your question, You should have ordered your GCMS notes since June, so as to check the assessments and see the level of works done on your application.
Also, you should have done your Medicals, that would've helped in concluding your application.

Again let me share my experience working on my daughter's undergraduate study permit which was approved in May may you will find it helpful.
She was already a first year undergraduate student in one of the universities in the SW. So she was given direct entry admission by the Canadian university - do not know what that mean though !. ( subject to submission of transcript of her study in Nigeria on resumption ) .

In filling the IMM 1294 form in the section where you are asked -have you attended any post secondary education including university etc? . You will tick Yes and fill in the year and month you where admitted to your current university and put to -date.
In the section where you are asked for employment history - You fill the same as above and continue to years of your stay in secondary school down to Primary/elementary school assuming there are no gaps.
In submitting documents we added her JAMB admission letter and course registration form as part of the pdf file in client information section This was just my experience and may not apply completely to your situation. The guru's will advise further.
