The result of the TB test is more important than the address to UKVI.
You should be fine submitting it just like that however if it is concern to you then you can request to change the address to the new school.i think there is a fee for it

Your school should be able to advise.
Some will require a reference letter others may send a link to your referees to fill the details in.

It depends and again your school will guide your appropriately

Would be best you cancel, and reapply. Better to lose visa fees than to get a refusal or even ban.
From the previous comments seems it is possible to withdraw your application anytime before a decision is made. But go to the uk immigration website & do your findings immediately

I am yet to get my CAS. I have paid the deposit and submitted the Student Route Eligibility Questionnaire for over 2 weeks but no response from University of Portsmouth. Although, I uploaded a refusal letter while filling the questionnaire. Could this be responsible for the delay in getting my CAS?

I think you posted this question multiple times

Anyway, regarding your US visa were you placed on ban? It seems so from the multiple refusals you have since gotten from Canada and Australia.

You have to declare this at the relevant sections during your visa application and to be honest it may or may not affect your dependant application.

2. you should remember roughly how many years after the first Canada application that you put in the second.Like @giselle237 said we can all remember or atleast estimate our applications and refusals. You could say for instance 2015 for first application and around 2017 (if you are uncertain)

3. The same applies to your Australian visa refusals, you can estimate the year. If you aren't sure of the reason, then state it as it is (i.e. unsure of the reason for refusal).But if you can track were you received a decision then please put the reason.

4. Your chances are 50/50, pray and hope this doesn't affect your application. My advice, your wife should apply first and get her visa

Finally do not blame the agent, I am sure you had an idea of what he/she was doing and approved it. It is okay, we learn, howbeit painfully.

you do not need birth certificate to apply. Your international passport is fine. If your parents are sponsoring, include birth certificate to show relationship.
