You don't need to state the reference no,

You can just give a summary of date(month and year) visa catergory and reason for refusal

They have the details to check in depth if needed.

Why cancel? is the mistake that bad? if you can avoid cancelling, please do, UKVI sometimes don't pay attention to some of the mistakes we panick about....

Secondly, can a MA apply a week before her dependant apply for theirs?
Does it have effect on visa delay for dependant?

1. If MA has her CAS and other supporting documents are ready she can apply, I think the decision making time is around 3weeks now and may be shorter by then

2. Yes the MA can apply a week before her dependant and there's no visa delays for the dependant because of that. The MAs GWF no should be quoted in the dependants application.
