Teaching hospitals and FMC are both tertiary medical institutions. However, the ranking is measured by the accreditation of the West African College of Surgeons, National Postgraduate Medical College of Nigeria and the National Postgraduate College of Physicians, to train medical doctors as specialists.
All federal universities teaching hospitals are by default approved by the postgraduate colleges for specialists trainings. FMCs can only meet the accreditation criteria to train medical doctors after getting at least 2 consultants, Junior and Senior Registrars in all specialties of the colleges.

Ideally, a Teaching Hospital will have a wider scope, as it is saddled with the task of raising new doctors. A FMC is just a General Hospital complex that is being managed by the Federal Government. It is also a tertiary medical institution as they give specialized care.

Doctors don't perform 'experiments' in teaching hospitals. They provide care that's taught as a side piece to students and other doctors.

If you want the highest level of care available in Nigeria a teaching hospital is your best bet. You're managed by a multidisciplinary team with people who have decade long experience.

Private hospitals lack that multi disciplinary approach, even at the best ones. It's unlikely that a diabetic patient will be managed by an endocrinologist, a surgeon, dietician, physiotherapist, nephrologist, cardiologist or more in the setting of a private hospital. Usually it's one or two doctors doing the jobs of these people.

That's fine if it's a simple uncomplicated case of diabetes, however when the going gets tough it won't suffice, hence why many private hospitals refer to teaching hospitals for many severe diseases.

I get your point about bureaucracy and inneficiency too. And you're right. Much like most government establishments in Nigeria Teaching hospitals can be annoying for patients. I would pick a private hospital for routine medical visits over a teaching hospital or any govt owned hospital for that matter.

That said you must be very careful in choosing a private hospital. Very very careful. Chose one you are sure has excellent medical practitioners, not excellent facilities and buildings.

At many private hospitals a lot of things are glossed over for plain ignorance and lack of experienced supervision (what you get in teaching hospitals) of younger doctors.

Nothing works there!! Avoidable Death Everyday in this Hospitals without proper Investigation. Which is Bad.

I lost my mother in that beautiful hospital. The young doctor was just disturbing my sister detailed to be with my mum. Last last, they saved a family member during Covid19.

University of Nigeria Medical center Nsukka is inside the university Campus, while University of Nigeria Teaching Hospital Ituku is an entire institution on it's own

Once again, sorry about your loss. However, we should understand that some medical conditions aren't as simple as we see them. Imagine wanting to clean up an house that has remained flooded for say 20 years in 2hours. It would have been easier if in the 20years, some maintenance measures to reduce the flood or provide continuous drainage were provided.

Health workers aren't magicians. The fact that he required an amputation is an indication that his blood sugar control wasn't optimal over the years and the diabetic foot he had would have existed for years, it progresses in stages which amputation is treatment for the most advanced stage. This is apart from the damages the suboptimal sugar control has caused in other organs like the kidneys, heart, brain, eyes, blood vessels. Most of which aren't reversible. If you did an autopsy, the primary cause of death would most likely have been from those organs and not the amputation.
You'll now agree with me that 40days to fix all those is just a trial.

I'll advise that you and your siblings start close monitoring of your blood sugar, blood pressure, and weight as early as possible.
Once again, do accept my condolences.

Federal Medical Center Jabi Abuja
FMC Jabi is far better than University of Abuja Teaching Hospitals.

Antenatal and Child birth Services, cardiologist, brain surgery his better at FMC Jabi than Gwagwalada Teaching Hospitals.

While Old people complications like Alzheimer, Cardio complications, Hematologist, Diabetes, etc. Use Teaching hospitals. Gwagwala use to be good then. But all those old experienced Professors and strong specialists don travel abroad. Jakpa

I used gwagwalada teaching hospital to manage my old dad's health.
I use FMC jabi to birth my Son.

A Multi-disciplinary approach to complex medical issues is definitely very important; by the grace of God I have never needed the type of care that necessitates experts from across disciplines to attend to me so I answered the question from a privileged perspective, granted.

That’s if those people actually exist in this country anymore. Apart from paediatrics, obstetrics and maybe cardiology, the level of specialisation is basically zero because all of them have ESCAPED. I saw somebody on tiktok lamenting that his sister can’t get treatment for a head injury because nobody in his vicinity knows how to read an MRI scan. And that was in Abuja for crying out loud!

I did not mention the calibre of staff or wealth of experience found in either institution because half of those guys pivot between both in order to supplement their income and build their portfolios.

As stated my concern is the abhorrent state of federal medical facilities across the country; even UNTH which along with UCH ought to be the gold standard for medicine and research at least in west Africa is shaping up to be a total disgrace. The last time I accompanied somebody there they wouldn’t even allow me to ease myself in the washroom without interrogating me like a criminal, is it supposed to be normal?

I think it’s short-sighted of you to dismiss the importance of good facilities in medical care; what exactly can even the best surgeon do in the absence of proper equipment? Of what use is cleaning and sterilising when clouds of dirt and dust billow in the immediate surroundings?

You asserted that the use of of the word “experiment” was erroneous but that sounds like a huge experiment to me!

What is “glossed over” at a private facility is what is not even addressed in a federal one. They can’t even buy a computer without waiting several months for a stamp of approval from Abuja,

You find people who are being employed as administrators standing outside gossiping on the phone and guzzling groundnut waiting for 4 o’clock to roll around so they can be on their way.

Why? Because nothing to do until CMD or DCMD or whoever approves it and department “Madam” hands it over to them, lol!

Most Teaching hospitals are federally-owned (not all) and the federal government uses a template whereby every state is supposed to have a "big" federal hospital which would either go by the name of a federal medical center (if there's no fed institution in the state or if the federal institution doesn't offer medicine or if there is a state hospital by that name) or a Teaching hospital (if it does). It seems the Teaching hospital in your pics is owned by the state government (not the federal government) since LASU offers medicine, too. Hence, the federal government still deems it necessary to create a "big" federal hospital in the state.

Edit: I looked at the images you posted again and it seems I am right. The Federal medical center was established (1964) around the time Unilag was created which hints that Unilag might not offer medicine at the time it was created. The federal hospital in Lagos state is called the Federal Medical center, not a teaching hospital. The Teaching hospital is state-owned.

In the clinical setting, consultancy is the highest cadre. Nobody actually gives a crap about you if you're a prof and not a consultant

And yes, Consultants can be HODs. If you're a prof and a consultant, it's an added advantage. But being a prof alone without being a consultant, you're nobody actually

If you're talking of Imo then you aren't up-to-date, as at 2021, FUTO began admitting for Medicine and surgery hence the upgrade of the FMC to teaching hospital. Technically they just changed the name, there's really no upgrades. FMC Umuahia and FMC Owerri had better service delivery that even UNTH in Enugu or Ziks TH in Nnewi.
