Guarantor's form is not necessary.

You need:
1. Application form
2. Payment receipt
3. Appointment slip
4. Declaration of age from a High court
5. NIN
6. Certificate of state of origin
7. 2 passport photograph.

The four things I listed under "For passport" are for passport application.

The two things I listed under "For visa" are what you need for visa application.

But I omitted two important things.

You need a guarantor for the passport application. A guarantor is someone who already owns a passport. For the visa, you need a letter from the company that wants to send your brother abroad. In fact, let me edit my main comment.

You need just slip for NIN.
