Well, I have answered this question million times.

The format will be in FORMAL

Your Address at the right hand side. Then the left hand side should be for the school. You will address it to the Office of the registrar.

Example below;

1, Biola Street,
Off Avenue Road,
Ikeja,
Lagos.
October 17th, 2014

Office of The Registrar,
University of Nigeria,
Nsukka,
Enugu.
