Yes it is really necessary. Even when they don't indicate that you should send your cover letter please always send it along with your CV. A good cover letter will get you an opportunity to be interviewed and see what you have to offer.

Well said plus there are some information you can't put into your CV so as not to make it unofficial. Such info can be incuded in the cover letter

You don't need to address to anyone. Just the usual salutation Dear ma/sir will do
