To your first question it's doesn't matter as long as ue surname and first-name is thesame and 2 ur second question Affidavit is available and I fink the school accept that so you can do affidafit for your birth certificate even state of origin. you will expose to more finz when u get to school.

E8 is not pass D7 is pass.. If yσυ havε̲̣̣̣̥ D7 in maths or English уσυя admission will be forfeited.. C6 is the least grade yσυ need to have i.e Credit pass.
