There a total of 11 forms listed at www.cic.gc.ca which are used for FSWP. I am applying for permanent residence under federal skilled worker visa program 2014. I am single and live with my parents. My parents are financially dependant. I have never lived in any other country except my country of residence for more than a month. I am not using a representative for my visa application. In my opinion I need to fill out the following forms:

1. Generic Application Form for Canada (IMM 0008) (PDF, 536 KB)
2. Schedule A: Background / Declaration (IMM 5669) (PDF, 286 KB)
3. Schedule 3: Economic Classes – Federal Skilled Worker (IMM 0008 – Schedule 3) (PDF, 191 KB)
4. Additional Family Information (IMM 5406) (PDF, 79 KB)
5. Fee Payment Form – Application for Permanent Residence (IMM 5620) (PDF, 27 KB)
6. Document Checklist – Centralized Intake Office (IMM 5612) (PDF, 233 KB)

and don't require to fill out the following:

1. Additional Dependants/Declaration (IMM 0008DEP) (PDF, 472 KB)
2. Supplementary Information – Your travels (IMM 5562) (PDF, 21 KB)
3. Separation Declaration for Minors Travelling to Canada (IMM 5604) (PDF, 21 KB)

Yes...you are fine with the forms.
Only add ''Supplementary Information – Your travels (IMM 5562) (PDF, 21 KB)'' and fill never travelled before on the column requesting it.

All in the name of having a perfect processing.
I did my diploma and Bsc in the same university. The diploma (satelite campus) had to be done to gain the admission then. Though it was done as a direct entry to the university.
I only sent my BSc to WES and it has been evaluated to four years equivalent. Mind you, I started from 200level in the university (3yrs in all).
Now, do I include my diploma along with my BSc with ECA in the package?
If yes, can I use notification of result? As I don't have a ceritficate
OR should I omit the diploma?

Just include the photocopy of the statement of result and you will be fine.

On the alternative,please try to contact your back physically and asked if there is a pending transaction on your account.
A buddy of mine was in same issue yesterday.It was at Gtbank that it was realized that there was a pending transaction from CIC on the account which needs the applicant's approval.
Once the approval was given,the account was debited immediately.
Funny thing is that the banks sometimes do not give approvals for such transactions and they will never notify applicants for such.When this happens after about 48 or 72 hours,the transaction will be denied.This has happened to me before and cost me alot of additional waiting time which is why I always advised applicants to go for demand draft(DD) because not all applicants find it rosy with credit card debit.
I really pray yours do not fall into this category....please visit your bank and explain further to a senior bank official to the one you have met before.

Paying through credit card in the link above will be N35,000
Raising a draft IFO of British council will cost you N38,000.....your draft must be N38,000.

In my original application package, I missed to mention the “Address of Payer” in the “Section A” of fee payment form. My fee payment mode is Credit Card. My spouse is the payer. Is Address mandatory to swipe card. What can i do now. I am a sep 3rd applicant. If i send a new Fee payment form, will they accept it?

Even though this not mandatory but you can send it to free you mind.
The most important thing for a credit card payer is the section B where the name of the cardholder and the signature signifying the authorization of the cardholder to deduct the required amount from the credit card.I'm sure this has been done for you.
Like I said to clear your conscience,send another as additional document stating all your personal details,NOC,Passport number,date of birth and any other information to trace your application.

Relax...it may take some days for them to update your details.
Why did you send it through NIPOST?...You should have send through courier.
Expect some delays on the NIPOST mail as you can not track it.
But all will be fine.

I have posted here before now about the inaccessibility of my WES report by CIC online. My evaluation was done in 2013 and when I got WES notification for a fee payment of $50.85 (for my report to be visible to CIC), I paid on 3rd October, 2014 through a link given me by WES.
Surprisingly, I got an email from CIC yesterday saying WES has not released my online report for visibility.
Is there anything you have to say to this?

I sent my MBBS and other documents to MCC since October 1st, and I got a email from MCC that the parcel was received by them through courier, and my status on physiciansappy.can has been indicated as ( IN PROGrESS) , over 3 weeks now.. I seem to have lost out in this FSWP.

Contrary to you,credit and debit cards are acceptable for CIC processing fees even though so many reasons avail that makes it unpredictable .....just that it affected me.
So many applicants in Nigeria have been successful including a recent experience of my friend i share with you.
My friend only gave a verbal confirmation/authorization without the so called pin code and it was debited within 10mins.
These are info you should have been aware of before using this card.......I'm sure the bank official is not well updated to understand what you explained to him or her.

Hello house & Hi damkollins. Congrats once again on your MR. To the glory of God too, I just got another job; and i'm asking if it is necessary to inform CIC now. I applied under 2131 and my new job is not related to it but I just had to leave because the pay nor be mate (x3 of my former). Also in respect to DoubleBam2x's post, i believe my type of assesment with WES can be independently viewed by CIC. Lastly, how was your medicals or you haven't gone? ( I remember helping my Uncle find his way to Yaba hospital then in 2012). As things stand now, you may not celebrate Easter in Nigeria.

If you don't meet up now...you will in January under express entry. Doctors are always in demand. How many of ur slots have they taken?

GTB issues DD in canadian dollars. I'd in Lagos, oyin jolasanmi branch. You will get it in less than 2hours. Good luck.

Gtbank,Plural house at Oyin Jolayemi is popular to get your draft done provided you have your dorm account.
Alternatively,you can initiate the FX draft from your internet banking and ask it to be delivered to the nearest Gtbank branch near you to pick it up or nominate a proxy on your behalf to pick it up.
In case your DD is unsuccessful,you can REPURCHASE IT BACK to your dorm account...it will be done within 10 working days.

I share your sentiments too. In fact i later went to GTB - where I also have an account- to discuss these same issues and it was a different story. I was told the direct opposite that the process is quite easy, the only thing is that they might hold the funds for a day or two but I'll still get an alert.

Now, I am thinking of writing to CIC to inquire about the status of my application. I think its against the rules to send multiple applications under the same NOC else I would have paid for a copy if my Ielts from British Council and obtained GTB DD to go with it.
Can someone please furnish me with the email address for specific case enquiries? I find it difficult to fold my arms and just wait for so long. Kindly advise.

I still believe strongly that u'll be charged. But still make whatever move u need to make, I understand how tensed u would be, I got my PER 10 days after others received theirs. My cc couldn't even pay for IELTS yet got charged by cic early enough.

If i am to turn to WES to evaluate only my ND and HND, how many points would that give me and how long would it take for the Report to be ready.

NB: All other documents needed for me to submit my FSW application are ready.
