So I did my verification today,and these are the documents needed for the verification Bio data,post ume receipt,admission acceptance fee receipt,o level,jamb result,birth cert,tax clearance,admission letter and local govt cert.PS you should do 2 photocopies each of these documents and arrange it as it is here.Don't forget to come with the original copy of each document.

Yes it is also called certificate of origin

NB:Pls if u don't have your passport on your post ume result, just go to the ICT and complain,You might be ask to pay a sum of 200# in OOU bookshop at motion ground,and go back to ICT with the receipt,if u don't correct it before going for verification, u Will be ask to go back to upload it.
