U said u r using FCMB right? The soft token given to you by the bank is only valid when doing transactions using their platform. It can not work with quickteller. Only the hard token given by GTBank works with quickteller. So u need to generate an OTP with any ATM Machine. Also make sure you input your CVV2(3 digits at the back of ur ATM card) correctly as it can also result in failed transaction
