The Real Reason Nigerian Hospitals Ask For Police Report In Trauma Patients by Nobody: 2:56am On Feb 26, 2015 |
You folks think the average roadside clinic can handle a gun shot wound or accident victim. They can't help the patients much even if they wanted to,they don't have the necessary equipment needed to do that, neither do they have the expertise for the most part. Even here in the USA ,severe trauma and gun shot patients don't just end up at the nearest hospital and even when they do it's just for stabilization before they are taken by mediflight to trauma centers equipped to handle such. You folks are demanding too much from people who have no knowledge,no experience and no training in handling things like this. Most of these doctors probably just have an MBBS and nothing more The facilities have no in house labs,X-ray or scan machines,no blood bank let alone more sophisticated equipment needed to stabilize such patients and certainly cannot perform the life saving surgeries these patients need. They operate with much limitation, truth be told Most are actually glorified dispensaries The police report is just a way to send them away to go die elsewhere and not in their facility The most they can do is put a drip in the patients arm if they can find a visible vein and watch him die The average Nigerian so-called hospital can't do much for you beyond malaria and appendicitis if you're lucky Let us remember the plight of DJDOLA ,two months in a Lagos hospital and millions of naira and they couldn't even give a definitive diagnosis and used our friend for experiment ,treating him for everything else and making up diagnosis along the way while billing the patient and smiling to the bank. That should be an eye opener 30 Likes 7 Shares |

babyosisi: Not all of them...but some of them. I once worked in a rural hospital(private)...where the doc in charge(my boss) removed a bullet from the thigh of a patient. Saved it for the police, who came in shortly after the op(My boss and the local police were on good terms). Even here in the USA ,severe trauma and gun shot patients don't just end up at the nearest hospital and even when they do it's just for stabilization before they are taken by mediflight to trauma centers equipped to handle such. Which is why we need to have an Emergency Medicine Residency Programme in this country....and why we need to train more trauma surgeons, and build more teaching-level hospitals. And it starts with increasing funding for health. The facilities have no in house labs,X-ray or scan machines,no blood bank let alone more sophisticated equipment needed to stabilize such patients and certainly cannot perform the life saving surgeries these patients need. Again, more reason why funding for health has to be increased. And we probably need more public private facilities. Maybe on a Build-Operate-Transfer policy.. The police report is just a way to send them away to go die elsewhere and not in their facility Not exactly. Sometimes the Police accompanies the patient, sometimes, they are just a call away. And sometimes....a police report is necessary. The most they can do is put a drip in the patients arm if they can find a visible vein and watch him die Depends. Many Nigerian teaching hospitals can handle multiply injured patients well. Where the problem starts is how the patients get to the hospital(no effective ambulance and paramedic service which means that patients are transported in private vehicles...wrongly)..and whether the patients can afford care.Even at govt subsidized prices....many patients cannot afford care. The average nigerian so called hospital can't do much for you beyond malaria and appendicitis if you're lucky I work at an average Nigerian hospital....and have worked in rural medicine....and I have treated many more cases than just Malaria and appendicitis. I have treated botched abortions(not the most pleasant bit of my career), diabetics, hypertensives, respiratory illnesses, HIV AIDS illnesses, diarrhea, cholera, skin diseases....viral, fungal and bacterial, the odd case of chicken pox, and many other conditions. And even working at hospitals in the rural areas that were not ''standard''. Let us remember the plight of DJDOLA ,two months in a Lagos hospital and millions of naira and they couldn't even give a definitive diagnosis and used our friend for experiment ,treating him for everything else and making up diagnosis along the way while billing the patient and smiling to the bank. Yes....largely because....zero facilities. And as someone has said here earlier elsewhere...the way we train docs in this country needs to be reformed syllabus wise. 71 Likes 2 Shares |

babyosisi: I was expecting to see a very intelligent and informed post, esp coming from the OP herself, but the above post is neither brilliant nor an informed one. The content of your post, no offence intended, is full of ignorance and prejudice. The National Health Law was only recently made a couple of months ago, making it unlawful to demand any paperwork before attending to emergent cases. Before then, it was not that simple. The Police would drag Hospitals and doctors into criminal cases they no nothing about. Hospitals had been listed as accomplices to crimes that happened far away from them, for merely "harboring" and treating suspected criminals with gun shhut wounds. There were many aspects to that, and while there were ethical guildlines to follow, ethics are not laws. Once, as a juniour doctor in a small private hospital in rural Nigeria, I had finished attending to a young man, polytechnic student, who had lied in the history that the massive and deep cut he sustained was from a domestic fight with a roommate. Moments later, while outside the hospital and, I saw my patient being excorted by many armed mopol and some anti-cult agents. I enquired what the problem was, and one of them harshly said my patient was "a cultist and a criminal!". I was not prepared to walk that lonely and long way to obtaining justice in Nigeria, cos they had no right to take the patient away in that health condition. I only alerted his relations. Next time, I was more cautious in admitting certain patients. Nobody expects a small general hospital to manage complicated trauma patients. Why then do we have specialists in trauma and emergency medicine, surgery and other spelciaties? But managing simple trauma cases, and identifying, triaging and stabilising serious cases is what every graduate doctor can and should do. Your comment that the police report is a simple way to send patients away to die elsewhere, and that the most they do is to set up drip if visible veins were seen and watch patients die, is terribly ignorant, prejudiced, regrettable and very harsh on the hospitals and the doctors and other healthworkers. I'm curious to know if this is how you project Nigeria in the USA? Your comments highlighting the inadequacies in expertise and equipment and policies in Nigeria, I agree with them. It isnt clear to me though, if it can be said that you have made them constructively. 93 Likes 7 Shares |

It's terrible that they are not honest with patients about their capabilities. Lives could be saved if people knew where exactly to head instead of being messed about. 2 Likes 1 Share |

The other very important reason they turn away bleeding and dying patients and prefer they die than help them is this Who will pay the deposit? When you pick up accident visits off the road and rush them them to a hospital ER in Nigeria ,often times neither you nor the dying patient come with bags of money and where there is no health insurance ,commercial or state run,who pays the bill? Even if people decide to carry around police reports in their pockets in the event of an unforeseen accident, the issue of thousands or hundreds of thousands if naira in initial deposit will not be overlooked. If the hospitals don't get you one way, they will in another. Either way,the man in need of emergency care is screwed Who wants a bleeding mangled up patient without a dime? 4 Likes |

somegirl1: Doctors in Nigeria are generally heartless They care nothing about their patients, human lives mean nothing to them and their nurses The reception has a standing order not to let anyone without the stipulated thousands get past the outpatient department no matter how sick All they are after is how much they milk from the ignorant helpless citizens A nigerian doctor will keep a patient for weeks and pour infusion ( drip) down their arms endlessly without a diagnosis Diagnosing your illness is not a priority, many don't care about that, they are after the payment of the bills That's why a patient with a treatable ailment will walk in with both legs and leave in a casket Butchers many of them In all my years living and working in the USA I have never heard of a single woman I know or a woman known by anyone I know die while having a baby but in Nigeria it's a minute by minute occurrence. People with normal uncomplicated pregnancies die daily at childbirth in nigerian hospitals due to negligence and malpractice and the families have no recourse. Ever wonder why these folks don't let patients in on what is ailing them? Ever wonder why they scratch off the names on the medication bottles they give you? Ever wonder why they get angry when a concerned doctor calls to chat about a patient in their care and course of treatment? The same corruption, heartlessness and greed that has permeated every sector of Nigeria is also rife in the hospitals Why should anyone be surprised? 21 Likes |

madam op, I tried to excuse you that this may be ur way of griefing-rants... especially when u feel djdola's case was unprofessionally handled. but generalising and mudslinging all nigerian doctors is beyond me.. I am particularly disappointed that it is coming frm someone with supposed medical training. worldwide there are 3levels of health care-primary, secondary and tertiary. you dont expect a private facility with minimum equipment to handle a poly trauma patient. and it is praise worthy a doctor is humble enough to refer patient appropriately. perhaps you hav lost touch with reality down here. considering the hostile work environment, insensitive govt., poor renumeration, archaic tools etc. a nigerian doc has to contend with compared to his foreign counterpart, me think they deserve commendations. yes, there are unscrupulous few that taint the profession; blame it on d bug of corruption and inaccountability that has plagued d country.it is not peculiar to medicine 17 Likes 1 Share |

babyosisi:Madam stop taking it personal abeg. You rant as if a doctor ditched u once. Its shameful if you are doctor or health worker sef. You stay in US and deride your colleagues personally without considering the whole nigerian system. So you dont have nigerian doctors practising in diaspora or what. Abeg park well joor. Hundreds die from accidents on naija roads, u blame bad roads, people perish in air crashes u blame the aviation ministry, yet a patient dies u leave the same system of chaos and curruption in Nigeria and blame the poor doctor who tried his best under the circumstances. Mind u naija docs are doing well worldwide, mostly. I'm ashamed of u if u are medically trained, cos u speak like an insolent ignorant lay person. How many blood bank did u set up in ur nigerian village so that pregnant people wont die every minute in your village. Cos am sure its in ur village they die every minute. Come down and practice in nigeria lets compare. Next topic jare 35 Likes 2 Shares |

My relative was a patient at one of the best hospitals Nigeria claims to have I could fill a textbook with the experiences,I have written some earlier You need to get a cooler with ice and travel miles to buy blood Imagine that? In 2015 this is medical practice in the giant of Africa Then you almost have to hover around the lab to make sure they are cross matching the right blood if not,you family member will die Being sick in Nigeria is scary Nothing works Most so called hospitals don't even have running water neither can you trust the gloves in the nurse's hands hadn't been used next door too The nurse is interested in their own protection not yours Patient falling off beds and dying from falls because nigerian hospitals don't realize safety should be regarded No guardrails on beds Of course no call lights to alert a nurse, not that you expect them to come anyway A patient in distress is on their own If no family member is around,he could be dead for hours before being discovered 13 Likes |

God help you in Nigeria if you happen to fall seriously sick before,on or shortly after any of the numerous public holidays Just make sure to keep a will handy and a casket too You may not make it The whole place is deserted, everyone is out to celebrate Do you know that in Nigeria, pregnant women have to register in 3 or 4 hospitals as patients They also have to buy a long list of materials for delivery and give to each hospital or give a money equivalent ,even though the baby will be born in just one Why 4 hospitals then?because when labor or complications hit, your doctor may be away partying somewhere so you better have a back up and three backups are better than two. Pregnant women with complications are rushed into emergency rooms of hospitals they are registered in and the doctor is 10 miles away at an owanbe and left his phone number to be called. The families have to run helter skelter from hospital to hospital to save the life of mother and baby Meanwhile doctor at an owanbe will not miss out on his plate of isiewu, let the patient die who cares,as he licks his fingers. They don't care,it's just a patient ,one patient. One young twenty something year old died years ago while I was visiting I went with my parents to visit the family on a trip to the village.she was married in the next village over but her parents had also placed banners announcing her funeral in our area and all I saw was a young beautiful girl smiling and happy on a poster she would have never imagined would be the ultimate use for her beautiful picture She was living in Lagos with her husband This was her first baby, she went for a routine prenatal visit, baby was overdue, husband at work, doctor checked her and decided the baby needed to be born immediately The girl drove herself to the hospital, car parked in the parking lot They wheeled the woman who was not in any distress according to witnesses into the OR,cut her open without the consent of her husband or alerting anyone and they killed her. Perhaps the doctor decided he needed the 300k delivery charge that day to roof his new mansion and a pregnant woman walked in and he say naira signs. Then she was brought home to be buried A girl with her parents and grandparents still living Dead at the hands of a butcher with MBBS and nobody does a thing 10 Likes |

^^ Her family could engage the services of a lawyer to sue the hospital and/or the doctor for medical malpractice, if there are any such laws in Lagos or at the federal level. What a heartbreaking story 1 Like |

Babyosisi, why all the un-necessary attacks on Nigerian hospitals and Nigeria as a whole? If I'm not mistaken, you said you haven't seen in the US where a woman dies while having baby, but I saw it in the news just last week or two where a lady died with the child. I used to live in Nigeria before moving over to Canada, and I can categorically say, most Nigerian hospitals are not near to any of what you've described. The system is bad, agreed. No 9-1-1, no emergency services, etc but it isn't all that bad. I live in Canada where people really don't get sick, unless for flu of course. You can't keep comparing a developing country with a developed country, the difference is just so clear. Anyways, only God cures, may none of our people have any course that requires them of needing emergency so bad (Amen). Enjoy your day! 27 Likes 3 Shares |

babyosisi: Well....that is largely because 1.Voluntary blood donation has yet to take off in Nigeria. 2.No light in our hospitals.....which you blame the Govt for. Then you almost have to hover around the lab to make sure they are cross matching the right blood if not,you family member will die Most of our lab techies get it right. And malpractice isn't restricted to Nigeria....it happens everywhere. And we docs always crosscheck blood we are about to transfuse. Nothing works All boils down to funding. We do not fund our healthcare well. And that is because of corruption, plus the fact that we have a dutch diseased economy.....which means we refuse to manufacture things, and have to import everything...which can be expensive. And no, most nurses do not reuse gloves in this country. I don't myself as a doctor. Patient falling off beds and dying from falls because nigerian hospitals don't realize safety should be regarded Funding. Low funding. Of course no call lights to alert a nurse, not that you expect them to come anyway I worked in a busy teaching hospital once. The wards had one nurse to thirty patients at night. Of course....we do not have enough doctors and nurses. Funding again. 5 Likes |

babyosisi: Well, I have worked on Public holidays and the hospitals where I worked were not deserted. You could still get ahold of the consultant if need be. Do you know that in Nigeria, pregnant women have to register in 3 or 4 hospitals as patients Some do, some don't. They also have to buy a long list of materials for delivery and give to each hospital or give a money equivalent ,even though the baby will be born in just one Truth indeed....due to funding constraints. Welcome to the world of limited funding for Nigerian Hospitals....where OAUTH Ife spends 11 million dollars as budget....while Mass General in Boston spends 750million dollars for research. And we do not have enough doctors and nurses. Why 4 hospitals then?because when labor or complications hit, your doctor may be away partying somewhere so you better have a back up and three backups are better than two. Not in any hospital where I've worked! Again, not in any hospital where I have worked!Anecdotes, anyway is not evidence. (now see me doing anecdotes....pot meet kettle ). One young twenty something year old died years ago while I was visiting Sorry for your loss. It is possible that (I am not an obstetrician, mind!) that she could have died from 1)Amniotic fluid embolism...which cannot be treated, even in a grade A facility. 2)Blood loss from a DIC due to an IUFD....3)Surgical error. What hospital was she operated in? If she was operated in a Private hospital with nary a Obs/gyn in sight....then that's malpractice. Again....we need more specialists/hospitals/etc. 6 Likes |

Our system is generally corrupt. No sense of emergency. Hospitals won't treat an accident victim without a police report. And even before u get the police report, you'd be asked so many questions that, if care is not taken, might land u in trouble; especially if it's a case of bullet wound. For these reasons, we've lost many lives as the good Samaritans are not bold enough to render assistance 2 Likes 1 Share |
