Secondly, are we to print the whole page,that is, the OAU yellow and blue theme at the top of the page incusive or should we just print out from the written part below?

you can print for as many banks as you wish, no penalty for that.....
2. What the bank needs is the part that shows: the chosen bank, payment type and the likes... The oau yellow and blue theme isn't required, you don't even need a coloured print-out.
