Tiababy: Newark New Jersey was awesome. Came in with wifey and spent less than 3mins with the cbp officer. Asked why we were in the US, told him vacation and childbirth. That was it. I was surprised. Took wife's pic and stamped us in. Went through Agriculture and also had to fill a form cos we declared having food and funds in excess of 10k. No biggie, no food siezed, money was not checked either. Just filled the fund declaration form and that was it 4 Likes |

Sometingud: My husband was with me so don't know about scheduling consent form Don't go to post office for Passport. Go downtown at the Passport agency. Google the address. I shipped my baby things ahead of time through friends and family going and coming to visit b4 I was due. The rest I paid excess liggage fees as it came out cheaper than using a shipping company for $1500. 1 Like |

Don't know about close to New York but in New York, I checked the following out "Kings County, Brooklyn, Queens Medical Center in Queens, Jamaica Hospital Medical Center, New York Hospital in Queens, I even went to wyckoff heights in Brooklyn and then finally settled for Mercy Medical Center in Long Island as their pricing was direct and straight forward (not so cheap but well, I have family less than 20mins to the hospital, so it was my best bet) For c-section,just have well above 10k -12k dollars... @ekweresplace, during my interview I was to go to Orlando Florida but I ended up going to New York. All I did was to get an hospital in New York and I had their bills with me plus I notified the embassy of change in my travel plans cos my visa was for vacation and I now intend to use it for childbirth ...I dint mention if I was changing states or anything. nefertitiram: Nah nothing wrong with post office but I doubt u can do expedited coz earliest is AT LEAST 2 weeks. But with the agency u can get it that day or the next day. And from her post it looked like she is pressed for time. But if u are in no hurry, u can use post office and just relax till whenever they send it. 1 Like |

I have made proper arrangements by contacting a hospital and a doctor in the USA for this purpose and we have adequate finances to cover all expenses. I understand that my Visa covers for the purpose but would like to request that you kindly update my records to reflect this change in travel plans. For those looking for information on how to get the consent form notarized in Nigeria, see extract from part 1 of this thread " Info on Notarizing for baby birth cert in Nigerian US Embassy: You just go on the website.. I think the link is on page 92. You book with your name and passport details. You pick a date and time just d way every other appointment is picked and print the confirmation. On the day you have picked.. You go with the printed form you want to notatize( fill the necessary parts leaving the signature and date section since you have to do that in front of the guy at the embassy). Also you need to go with your passport. You get there, you should be directed to the US citizen section... Get in and pick a number... When ur number is called... Form and passport is collect from you(Note: it's free). So the guy calls your name when it's ur turn and he'll probably be gisting with u and all while he ask u to sign and he gets the form notarized and you are out." So for those whose husbands won't be around after delivery, ensure he gets the consent form signed off in Nigeria at the US Embassy and DHL it to you within 48 hours after delivery. This would ensure that you get the form by the time the BC comes out. Please note that this can only be done after you give birth. However, for those who want to process the Nigerian Passport /Visa by theirself, here's a sample of the consent form I developed for my baby. "DIRECTIONS FOR COMPLETING THE VISA AND PASSPORT APPLICATION PROCESS YOURSELF (not using an agent) Sample of Parental Consent letter Parents Names, US Home Address, Date Month 2015. To: Consulate-General of Nigeria, 8060 Roswell Road, Atlanta, GA, 30350. [or use the Nigerian Embassy closest to your accommodation] Dear Sir, Parental Consent to process Nigerian e-Passport (or visa) for Childs’ Name (infant) We, Fathers’ Name and Mothers’ Name, the parents of Childs’ name , wish to obtain a Nigerian e-passport (or Nigerian visiting visa) for her/him and hereby give our consent for your office to process a passport(or Visa) on her/his behalf. Please find attached the following documents - Completed and signed e-Passport Application Form - 2 recent passport photographs of child - Payment and Acknowledgement Slips - Parents Bio-data pages - Copy of child’s birth certificate - Processing fee of $30 in money order We deeply appreciate your assistance and thank you in advance for the usual swift co-operation in this matter. For those in Atlanta, use the below information to process your baby US Passport. " For the US Passport: If you want to leave the US on time. Call the Passport Agency on 1 (877) 487-2778 to book an appointment for your US passport and head to The Atlanta Passport agency, 230 Peachtree St NW, Suite 1000, Atlanta GA 30303 on the scheduled date. Have your application form, baby's BC, passport photo, flight ticket, etc handy. You will be advised on what is needed on phone or you can check http://travel.state.gov/content/passports/english.html for details. Your passport can be ready same day if you tell them it's urgent. Max 3 days. Costs about $165 expedited. " I take a bow. Please lets all try and take time to read through both part 1 &2 of this very detailed informative thread. queenestty: Lol relax! U are too stressed. Letter and correspondence from Dr doesn't guarantee entry. I traveled with no letter or choice of Dr sef. And only 40 dollars in my bag becoz hubby had wired all the money to his Bank of America acct. I just had his Atm cards. And I told the border agent that I have a Dr and hospital in mind and will decide when I visit both. He said ok and stamped me in. I made up my mind when I was settled in Atl. It's all about confidence and how u answer their questions abeg. I no carry any burden of proof oh. Atl obgyns will not give u an appointment till they get ur records. So u can send it to them if u want appointment but they won't send u any letter or correspondence. |
