You'll need go to cafe to print that.The info u need are jamb reg no,surname, email add and phone number.You'll need print that if you're paying in bank but if not bank,you don't need pay advice.I.e online payment.

I'm very sure ur card is a visa "debit" card don't bother urself because it won't work, just save urself frm tinkin, go to the bank close to u, I bought my form from zenith bank, if u stay close to berger, use d zenith bank there, if u check banks arnd ur area and they dnt hve it, dnt waste ur tyme, go to unilag and get d form, simple

n print it from any cafe, submit it at d bank
