nope.. In the screening form,they don't even look at the school you attended at all.. All they are interested in is no 1-8 which includes ur name,course,score,reg no,state&LGA,and date of birth. So,u are good to go... Fire on..

mine is just name issue! U dont need d biodata yet, just screening form and the one with 'full names of desk officer, items checked by'. Then ur admission letter from jamb and ur jamb result slip
