from my understanding there's a localised gene on the y-chromosome that triggers d formation of testes nd other male features but it doesn't form it by itslf. so in the absence of this, d individual will be female. where my point is, is here. there are factors that can silence this gene, the totality of these factors i don't know. however, it is clear that they also pay important roles in sex formation. moreover, there is another gene that can reverse male differentiations as found in rats. i think it's called sox9 gene or so.
i also think if sex determination is solely based on the sex chromosomes, the rate of the appearance of ambigous sexes wouldn't be put @ 1 in 20000 individuals by researches. i believe such rate of 'mutations' would be too high for such developed animals as humans.
