u need jus parent/guardian consent form, i.e whoever u stay wit should do dat but dat should be wen ur name appears on d 50 man list (AFSB), signing of form by a C.P, or som1 around d rank of LT.Col and above or equivalent belongs to the Regular Recruitment Intake (Depot).

guy there is a local goverment attestation form that should also be signed by a lt. Col. Or its equivalent and above in the navy and airforce or chair. Or sec. Loc. Govt. When u get shortlisted into the NDA 50 man list.

your acknowlegdement form does NOT contain your EXAM center. It is your Exam card that contains it. You will print your exam card when the portal closes. And only those who scaled Jamb will be able to print it.
