i think bio data will fill auto. i tried to register for courses buh it needed health reg .. now i need health number

do I choose my health center number myself or will it later be given to me by the school

I'm scared of jst writing anything cos there is a health biodata form to be printed nd I don't think we will need to print it if it won't be needed in d school, what if the health center number is later given to us in the school after filling a wrong thing on the form nd what if we re asked to later submit the printed form, will there not be complains or problems because of the wrong health center number?
