for that **smh** I remember ticking a disease/ailment/an health condition that I actually do not have, just in the quest to get the form COMPLETELY filled. Oh, yeah.. and the dates I received some immunizations that I in reality never received

Have you been able to get it done? If no. Go through the sections A-F, then follow the instructions and layout. In section F specifically you'd see a list of courses you could possibly register for. The first 6 are the core courses (LIB001 inclusive) and the rest are the electives (you'd see about 5 of them). You can choose 4 electives max, 'cos each elective is 2 units, thus the add up of your core courses (15units) and 4 electives=23units. I hope you find this helpful.
