JAMB DE Registration Slip Print Out (I.e. The slip you printed after the DE registration),
and either of these two:
Birth Certificate,
State of Origin Certificate - Note: you can go with both in case of necessity.

1) DE slip (DE print out)
2) O'level result
3)Statement of result from ur school
4)Local government state of origin certificate
5)Birth certificate/court affidavite

NB: the first 3 are primary,that is,very important,then if u have both 4 and 5 better,or u should at least have either of them

No need for another regularisation. since you have your records updated and your admission letter printed. keep your transcript, too. but be careful with it, a tear or much stains can render it useless. Morning.

You don't need to do Another one next year again, just make sure u validate d pin on jamb site @ any cafe nearest to u.

Welcome bros,concerning ur question,I don't know for sure but guess what,I will not advice my enemy to do that,so try and submit ur credentials if u have them with u.

These are my advice for u:
