greatie: Yes greatie. I did mention that the form is valid for three months from the date of seal however it is also important to mention the following A) d form requires the babies date of birth and unless you are having a scheduled csection (in which case even baby may still come before scheduled date) you cannot assertively tell date of birth. B) if you decide to leave the date of birth blank as has been mentioned on the thread, then the notary shouldnt legally sign it because he cannot attest what he does not know... And please lets remember its ilegal and not encourage it. A notorised form is simply an attestation (leagl oath) that the details contained are true. Falsifying such records are criminal and can be prosecuted. C) some states do not accept consent forms notorised outside the US consulate. Please note my intention is not to scare anybody but in d spirit of truthfulness and redeeming Niigerias image, i would advise against pre-filing. mamamj: My advice: Please ask your hubby to visit the American Consulate in the country where he would be at the time of the childs birth. As long as he is legally present there, he will be attended to. His international passport and visa or entry permit would be used as Identification. 5 Likes |

mamamj: Although some judges are notaries, not all notaries are judges. I personally advocate going to the embassy for the following reasons a) TS FREE b) some states do not accept consent forms notorised outside the embassy. So u may have to inquire about the practise where you are. c) where an error exists on d form, they are more likely to still issue a temporary passport based on authenticity by the embassy. d) d document is considered genuine and unquestionable |

larrytoks: I applied first week in May (not sure of the exact date) at another branch and I was called on may 31st to go to yaba branch to pick it up. Hospital bill was paid through another bank. |
