if you don't know there's what we call polypharmacy and if a particular brand is not available it can be substituted for the available brand. It is not even ethical to prescribe a medication using the brand names, you prescribe using generics. The OP is human and any human can react to drug. that's why your pxcist will tell you that all drugs are poisons, only the dose and your genetic makeup helps especially as regards drug reactions.

Seems the fellow involved finally divulged further info: he walked in and requested for Ciprofloxacin and was educated and offered azithromycin.
A pharmacist is the custodian of drugs and their appropriate use. Pharmacists also by their charge educate patients and promote responsible self medication, health promotion and rational drug use. However, the pharmacist shouldn't have condoned irrational use of an antibiotic given the current drive for antibiotic stewardship to reduce antimicrobial resistance.
A cursory look through appropriate clinical resource (central library of medicine- pubmed) however indicates that in asthmatic patients, Azithromycin use leads to lower primary end points i.e. reduces symptoms and exacebations. The best course of action would have been to discuss this with @Demilader's physician and seek an agreement to pursue this course. Barring an idiosyncratic reaction, the drug should actually have made @Demilader feel better. Pharmacists are able to provide generic substitutions for patients without recourse to the prescriber but are always required to discuss therapeutic substitutions with the physician (in practice settings where prescriptive authority exists for pharmacist prescribers, a short term intervention can be taken without consulting the physician but must be documented for review within a stipulated time).
I do hope @Demilader lives well and heartily, and wish we would all just look at the facts and make progress. This incident should ordinarily have been documented and a proper clinical and regulatory review done.

He can offer anoda brand quite alright and anyone can adversely react 2 any drug ,bt in ds case d drug in question (d particular drug he offered) had a known contraindication wc I expect him 2 knw as a pharmacist,buh he didn't do d needful by asking a question shld hv askd b4 giving d dude d drug...If u cm 4rm d angle dt he is jes human n is prone 2 mistks(maybe he forgot 2 ask d pertinent question),its understandable...Wot i don't agree wt, is trying 2 shift blames (eg "its not his fault,d dude shld hv told him he had asthma,bla bla bla..." )

Your knowledge of hospital set up seems to be very limited if you have any.Nobody calls the attention of the doctors to prescription other than pharmacists.They question and validate prescription before dispensing.Nurses call the attention of doctors to prescription errors out of their initiatives not out of responsibility.It is exclusively the responsibility of a pharmacist.That is the reasons why doctors love nurses than pharmacists in the hospital.
Lol. This is not about who has 'exclusive' duty of calling doctors attention or 'correcting' the doc as some will prefer grin. Anybody and I say ANYBODY including the clients themselves can do that!! A nurse called the attention of a very senior resident to make clarifications about a patient's prescription last Friday and I heard when the doc thanked her. What's this alien sense of entitlement about? This is strange to our culture of medical care.

this dude is one stubborn he goat his parent should have told him that anyone can react to drugs. from his writing it's possible the dude is a doctor who is frustrated and has something against pxcists. just like I said earlier, you will still need a pharmacist today, tomorrow and till death do you part from this world. even in the mog you'll still need pharmaceuticals.

The problem was not taking history and the victim here not volunteering such information.

Pharmacists are actually the professionals that advise doctors on the side effects of drugs. Pharmacist spent years learning about interactions of drugs with patient not doctors.

In the US, the FDA which oversees all new drug applications, depends on clinical trials conducted by the "drug companies" to determine the drug’s therapeutic advantages and disadvantages.

Maybe things work differently in Nigeria. You can google the articules below.
Just had to comment, couldn't stomach it anymore! what you have up there is grossly inaccurate. Where in this world is there a doctor that knows nothing about the drug he is giving his patients? gosh! if u want to be bias pls go right ahead but those screen shots are untrue. it's even illogical.

It is normal to have adverse reactions to medications.

This guy is making unnecessary noise.

If you read the side effects of some medications (some worse than the original ailment) sometimes you will be forced to avoid taking any medications.

Everyone reacts differently to medications. He is being ignorant.

There are idiosyncratic reactions to drugs. Sometimes people react unusually to drugs and it is not the doctors' or the pharmacist's fault.

The Azithromycin that was given to the guy was not supposed to cause status asthmaticus (in fact, it was supposed to prevent it according to some research).
It could be that the asthma attack just coincided with the time he started the medication and he is now putting the blame on the drug (and the pharmacist)
Certain things can trigger acute asthma so we need to know all the circumstances surrounding the attack that he had.

It's now about you comparing nurses to pharmacists. have you ever heard about pharmaceutical care? that's the in thing in the world and Nigeria is not being left behind not to talk about the unit dose systems of dispensing. stop being stupid be you a doctor or a nurse. so a nurse knows more about drugs and can identify medication errors and potential dangerous medication interaction better than the pharmacist?
I will never fill a prescription I noticed any form of error and I don't call the nurses attention. lf i noticed a problem on a prescription, I alert the GP and if he insist i fill such, I don't cos I can be sued for negligence by my patient. stop misinforming the public. you are probably scared about the knowledge the pharmacists has.
i pity your generation of professionals.

I'm aware those who are educated in character and learning present their arguments with decorum rather than validating their foolishness in the public by resorting to abuses and insults.
I'm equally aware that the best prescription for oral diarrhea is silence.

No doubt, pharmacists are good... what if he prescribes d right drug for a particular ailment, buh contraindicated cos of a co morbidity the patient has Doctors do extensive studies as to this regard because it can send a man who is hale and hearty, to his seemingly early grave. Thank you!!!

Are you saying there's no collaborative practice between your pharmacist and your doctor? pharmacists produce the drug your doctor prescribe for you so don't react as if they no nothing. who knows maybe you visited a patent medicine dealer. pharmacists know lots about medications than your doctors. No pharmacist will ever give you a drug you'll adversely to because they know so well about drug reactions.
Before you take any drug make sure you talk to the pharmacist. They are the best resource you can ever get on drugs.

This should not turn to pharmacists vs others bashing thread though!

The information given by this patient is vague. No name of drugs initially prescribed and eventually given... No information about the Pharmacy premises etc... How do you expect us to assess and believe all these craps. Pharmacists are trained experts in drugs and you can't take that away from them. There are countless occasions where they have positively helped correct or modify Doctor's prescription that has helped the patient. Why bring this kind of post to defame a profession?

And wat happened to directing the man to check another pharmacy? paracetamol and ibuprofen are not always alternatives to each other, as the patient may have peptic ulcer which rules out ibuprofen. u dont give alternatives wen u dont have patient medical/drug history.This is just greed

what do you know about drug interactions? when we talk about drug interactions, you should know we have different types Viz; drug-drug interaction, drug-food, and drug- organ interactions and drud-disease interaction. there's this course called ethical dispensing in pharmacy school and screening for drug interactions is one of the components of the course, there's no such thing in medical school so stop saying what you know nothing about. even the pharmacology and toxicology they do is taken in pharmacy faculty by pharmacist. The pharmacists knows everything about medications more than any health personnel and real doctors know that but not the ones flaunting ignorance on nairaland.
Pharmacology covers thesethigs and the knowledge is on the net as well.
Doctors are aware of them.
Pharmacists only know about the chemical structure and drawings more

Are u saying they shud rather cover up such unethical practice by some pharmacists who dont know their boundaries? patient management is never the duty of a pharmacist, they only dispatch drugs prescribed by a doctor and according to the prescription only. Any modification is fraud
