I did not read the first post, but thank God you are okay. There are so many brands of cipros that you buy from pharmacy only to find out later it is fake. The pharmacy themselves would not know the medicines are fake. My sister was given cipro tabs last month and took it for two days and that would have been a disaster becos she survived by the whiskers. We should query cipros whenever is given to us.

This whole episode shows the federal govt needs to put laws in place so only doctors can prescribe and chemist cannot dispense without a valid one. Except for mild painkillers and other mild drugs

Actually, for that condition, the alternative the Pharmacist gave is better. The guy only developed hypersensitivity reaction to the drug. And get it, that type of reaction is not limited to the alternative given, any other drug could have triggered such reaction. The delicate thing is, you won't know the drug you are sensitive to if you have not taken it or its equivalent

This logic is skewed, so, because some Doctors make mistakes viz a viz prescription errors and wrong diagnosis, they should not be allowed to diagnose and prescribe again?

If that would save lives, then that should be done. Nigeria is not ripe enough for pharmacy practise as it is being done in advanced countries. A result of the decay in our academic training instituition; probably why a lot of Nigerian trained pharmacist find it difficult to obtain the licence to practice in developed countries.

That's exactly the same shirty argument given to the NUC by the MDCN when the issues of pharm.D came up. Nigeria is not suitable for advanced pharmacy practice but medicine is. You are now the one to determine the quality and standard of education to be attained before one is licensed as a pharmacist? you also write the same qualifying exam pxcists write when they go abroad so what's the heck about px education? are medical schools better than px schools in Nigeria?
There are thousands of nigerian pharmacists who are licensed and are working abroad. stop saying trash!

That's what the medical doctors hate and why they are fighting very hard to scuttle the Pharm.D initiative but it's already late. The nod is being given by the NUC already.

I don't think medical doctors hate or oppose pharmacists making inputs regarding the best for patient management.
But making certain diagnosis and planning course of management are areas in which medical doctors are specially trained in.

Based on assumption, the fellow involved had just an upper respiratory tract infection and approached the pharmacy to purchase ciprofloxacin, it could have been on self-prescription and the medication recommended by the pharmacist was more appropriate.
But 'spot-prescription' for patients based on symptoms is not the best and may be tragic.

If the patient came in on self-prescription, it would have been the best to advise him to seek proper medical evaluation with evidence-based management - the cough or chronic cough he had could have many other causes other than an 'acute infection' and which antibiotics would not have managed.

The occurrence was obviously an adverse drug reaction which may have been revealed during a proper medical evaluation and avoided; even if the patient had not experienced such in the past, no one would have been held accountable, unlike what's happening now as he is being compensated by the pharmacy. In saner climes, that could have led to a law-suit with grievous consequences.

You say perceived mistake, you obviously don't know what Status Asthmaticus mean. Its like euphysema for the young. I have a sister that had Asthma and I can relate. 29hrs man... He has every right to be angry.

Ive been diagnosed (misdiagnosed) of asthma and I'm a pharmacist so yes I do know what status asthmaticus is. A whole lot more than you even. The issue is the pharmacist is not culpable for his severe reaction to an otherwise choice drug in his condition.

Circa 1 out of 10 suceed in the qualifying exams. What does that tell you? Why is there a high failure rate. These are same exams where Egyptian trained pharmacists excel and have high success rates.

Same cannot be said for Nigerian doctors. They habe an high success rate in qualifyinh exams. The small city where I work has 20 doctors out of which 13 are Nigerians!

Maybe PCN/PSN/Pharmacy Schools needs to review the pharmacy curriculum in Nigeria to better match what is obtainable outside the shores of the country. Thank you.

I do understand that most nations have switched over to the pharm.D degree but over here in Nigeria we are still running B.pharm. Thank God it's already being given approval by the NUC. If not for opposition from the medical doctors and the ministry of health pharm.D would have become a reality by now.
We hurt the patients because of our selfishness. Nigeria and her people have lot to gain from the implementation of the pharm.D degree but no one care since we sacrifice productivity on the alter of sentiment in our country.

The pharmacist is very wrong and that is that. He had no rationale changing the prescription. For crying out loud dats a different class of drug.
y would u change a drug without taking cognisance of d sputum culture n sensitivity pattern? And Hello he's got allergies.
He came with a prescription sheet NOT for advice.

Lesson learnt. Sell ya market not changing prescriptions.

to all the pharmacist here, who in defense of there colleague claiming that Azithromycin is better than ciprofloxacin ; do u guys know anything about antibiotics stewardship at all.
the drug reaction is certainly an idiosyncratic reaction that would have occurred with any drug,even cipro . But no pharmacist have the right to change a doctors prescription without informing the physician

Just look at what this one is saying. So if a doctor requests a throat swab or sputum MCS and the culture and sensitivity comes out with sensitive to ciprofloxacin and resistant to others and it is prescribed, the patient comes to you and with your quackery you will refuse to dispense the drug? you see how ridiculous you sound?

A pharmacist has no right to change any medication for a patient without recourse to the the prescribing physician. you are not privy to the patients history, physical examination findings, laboratory and radiological results as well patients socio economic situation all of which influences drug choice.

And for your information Mr Pharm Doctor, Ciprofloxacin can be used to treat both upper respiratory tract infections like sinusitis and lower respiratory tract infections including nosocomial pneumonia.
