I will advise you to withdraw via PayPal in the interim. Use an exchanger if you don't have a US PayPal account ( you can always modify your PayPal email later), and when your Payoneer card is available, you then link it to your Fiverr account and withdraw to it subsequently.

Contacted customer support and I was asked to provide an ID which I did (my Nigerian International passport ). They have contacted me to explain the difference in location on the account and the ID. My worry now is :
1) What do I tell them ?
2) What are the possible consequences ( e.g Is there a possibility of losing the account ) ?
3) What happens to my $$ which I am yet to withdraw ?

As soon as you withdraw the money into payoneer, they will remove $29 for card maintenance. This will be done yearly. And you can't sell your funds to an exchanger until you've made transactions that is up to $200. They also charge you $1 per withdrawal (money appears in your account after 3 days) or $3 per withdrawal (money appears in your account immediately).

With paypal, you can withdraw and sell to an exchanger immediately as long as you have up to $20. I'll advice you wait till you have bulk withdrawal before using payoneer. But if you're not in a rush to use d money, you can be withdrawing to payoneer until it's up to $200 then you sell to an exchanger.

I think the 3rd option is for only US residents with a bank account.

did you try the 6 combinations I suggested?

The only thing you can do (apart from trying out the combinations) is to tell them that you are a Nigerian living in the States but you're back to Nigeria and will be leaving for the States by December.
