Nigeria's Health Minister, Isaac Adewole, has announced plans to launch a new effective primary health care system in the country.

There'll be an overhaul of the previous system, he said. The move could lead to primary healthcare centres, PHCs, “having full control of how they utilize the money” they are allocated, he added.

“The administration of President Muhammadu Buhari would love to be remembered in making basic healthcare services qualitative, accessible and affordable to all Nigerians,” he said.

“Financial autonomy at the PHC facility level, making sure money gets to the primary healthcare facilities directly with facilities having full control of how they utilize the money, will help enhance the viability of PHC in the country.”

This approach, according to him, “informed the budget process in 2017 and underpins the approach for increasing healthcare services as espoused under the approved guidelines at the basic health care provision function.”

I sincerely hope for this to work. Health is wealth, and doing this might be the only legacy of this administration.

Nigeria lacks the technical, financial and political sophistication and robustness required for a complete decentralisation of health services. To streamline the health system, it may be necessary to bring the primary health care (PHC) facility under the federal roof, and add tertiary care to the responsibility of state governments.

Right now, the decentralisation policy which makes local governments run primary health care in Nigeria rests on the imported notion that services are most efficient when governance is close to the people, an assumption that is premised on the existence of a well-functioning participatory democracy where the electorate are neither hungry nor ignorant.

Most of the rural people our PHC facilities serve have not been exposed to high quality health services so they accept what they get as the norm or, when they imagine it not to be the norm, without complaints. When they cannot put up with low quality services they ignore the PHCs by staying at home, and they consult quacks, only to present in the PHC or other hospital in emergency, often too late for life-saving interventions.

As suggested earlier, if the primary health care (PHC) facility is brought under the federal roof, and tertiary care is added to the responsibility of state governments, the role of supportive supervision can then be left to the local governments who will function independently with verification of their activities by civil society.

I am afraid this proposal may only look good on paper. Implementation in reality will be difficult, and there are great political hurdles to reorganising a system, especially when such reorganisation involves huge financial commitment by the different tiers of government.

The Federal government should have no business in primary health care. States are more importantly LGA's should be in charge of healthcare in their domains. We need to stop this "one shoe fits all" approach.

Are you in the system? If no just relax. If you are in the system then you know he's more worse than dalung. Workers under his watch haven't been paid some since August and so on.
He comes up with propaganda and counter propaganda and has achieved nothing but mere showboating.

Pls tell that to the dogs. I'm an insider, when you withold funds meant for various projects with the hope of sharing it at the end of the year, that person is evil. In your own unbiased assessment how is nafdac doing today? Agencies under the health ministry are stinking with corruption and occultic practices like never before. Adewole has been terribly compromised
