First off, the guy messed up big time by concealing this from your sister. The marriage started off on a wobbling foundation. He should have had a real heart-heart talk with her about his challenges.

That said, your Sister's hubby is obviously suffering from acute erectile dysfunction. He needs to visit a Urologist. There are quite a handful of treatment options available for him including PEDs (vega, cialis, viagra etc), pens pump, penis ring, etc.
