2. Yes medical report is compulsory... It wasn't for my batch but as a result of the sad incidents that occurred in several camps they have made it compulsory now

Statement of result or certificate (whichever u have)
School I.D card or letter of identification
Passport photographs (at most eight)
Five(5) photocopies of each... If you do more, you'll only be wasting your money... Trust me.. You might not even use the whole 5
