You don't need all the forms for the exam, those things are for the interview stage, the only ones you need are:
-parent/guardian consent form
-local government attestation form.
-acknowledgement form ( I don't even know what this one is)
-photocopies of all credentials including nysc discharge cert.
-pencil, eraser and sharpener...

It was clearly stated in that page where you downloaded the list from

This is same thing as indigene certificate nah, your local Government ought to have representatives in every state nah, they usually have those things....

please does the police certification form fall into the acknowledgement forms?

Also the Local Government attestation, they said a senior military officer from the rank of a commander or equivalent sign or the local govt chairman.
My question is, I am not close to my state of origin and also where I reside is far from my the state capital of my place of residence. Can a Lt col or colnel in the Nigerian Army that is from my state sign for me? He asked me to come for the signature tomorrow.

Commander or equivalent means navy commander or equivalent of that rank in the sister forces. That'd be Lt. col of army or wing commander of the Air Force.

Gentlemen good evening

I can't but laugh at all these questions and confusion.

Instructions are clearly stated, some questions shouldn't be asked. However, let me help, with my little experience, clarify some things.

1. Whoever should sign whatever form is clearly stated on the form. Meet appropriate persons to sign.

2. Go to your exam center on Saturday with all your printed out forms, signed and photocopied. Along with PHOTOCOPIES of your credentials.
You may or may not have to tender them as it wasn't demanded last year. But be safe and smart.
Again, photocopies of credentials not originals. If you go with originals and misplace them in the middle of running up and down for exam, you're on your own.

3. Don't think it's going to be calm and straightforward, you'd enjoy he military chair for a while on Saturday. But keep your calm and stay focused.
And let me clear your doubt, calculators won't be allowed for the exam.
Follow strictly all instructions stated on the website

4. There's no dress code. Nonetheless, dress decent in Clothings that allow for easy and free movement. No need to shave at this stage.
Eat well before going for exam.
