kitone:its not about training it’s about career progression. A nurse’s salary should not be clipped all because she should not earn more than any doctor. My relative in the US is a registered nurse with over 20years experience and she earn more than some doctors, although the doctors will eventually earn more within a short time as they progress, it doesn’t stop the fact that she’s climbing up. In fact she corrects and even tutor some young doctors on the field. It’s only in Nigeria a fresh graduate doctor will be feeling like Ben Carson. 3 Likes |

That patient in aggregate is the one who should decide how pay is allocated .That is capitalism If I have $1 billion to invest in a hospital would I make a doctor the CEO? Doctors have no training in budgeting,Financial Accounting,Management accounting,Operations Management,Industrial relaations,Workforce planning,Public administration,corporate law etc. Nurses have more management training and experience than doctors . They manage a larger number of staff and relationships which make them better suited to becoming Hospital Manager and CEO. The person who I would employ does not even have to have a health background at all. The moment we understand this in Nigeria we can start moving forward. doctors are entitled to insist on RELATIVITY based on their own experience of the world. Whether they get that is down to the market. I do not believe doctors should strike. If I was in a position of authority I will stop all these strike permanently. My solution. Sack them all, let them organize and employ themselves in cooperatives and sell their services to government. Then cooperatives can compete against each other ,that is capitalism. This communist approach of drawing remuneration of unclear basis is outdated. In my regime we will develop metrics to measure the financial contribution of each individual worker and that is what would determine pay. There are hospitals in this world that do only one thing e.g hip replacement or herniorraphy all day long 7 days a week. A surgeon can work as much or as little as he likes,there is always work. If you want to do 1 day fine you want to do 7 OK. You are paid PER PROCEDURE and penalized if the work has to be redone. That is how it should be not talk about what you did to qualify.Who cares? The issue is how much is the market willing to pay for a hip replacement or herniorraphy. The same would apply to other professional. So if you do 10 herniorraphies a day there is a limit to what you can earn based on what the business can charge per procedure and its other costs. A doctor performs brilliant operations but all his patients die because of fake drugs/materials or poor infection control of what use is his brilliance. There are others whose job it is to ensure drugs/materials are genuine and infection control optimal. Without them the doctor fails so who is more important? In my view no one. So who should get more pay? This is determined by demand and supply and not by any intrinsic superiority as doctors like to pretend. I have seen some specialist nurses and technicians earn more than professors because of workforce shortfall. If such "hgh" pay is sustained it will attract new entrants into the profession, demand and supply would change and a new equilibrium would be found. This is an endless process in the market. Overall the equilibriating process seems to favour high wages for doctors but it is easy to wage war on that as a country by creating a new class of clinicians that are cheaper to produce over 2-3 year who can do a lot of what doctors( Medical officer class) do especially non-surgical conditions. At the specialist level there is no competition but at the Medical officer level quite often others who are not doctors can be trained up at much lower cost. To my mind any doctor who does not progress beyond MBBS is a waste of space, One does not need to train for 6 years to do what MOs do especially in internal medicine in Nigeria Let the market not strikes decide and the way to do that is to privatize. Then if one group is not happy they can quit The sooner the government removes its hands from employing doctors and nurses the better. THEN pay would be determined by HOW MUCH MONEY followed YOU into the business not how hard your degree is. Your economic value as a worker is the value the MARKET places on you not how hard it is to get your qualification. Whether a goat eats grass or yam and egg . The idea of doctor as "head" of medical team is one many doctors fixate on. The truth is that that is usually the case but it is NOT ALWAYS the case. Many physiotherapists do their work without any medical input or supervision and are entitled to their own professional independence. Outside of clinical scenarios as far as being CEO within a tertiary health institution is concerned,there is no case for doctors to occupy this position automatically. The position of Chief Medical Director as we have in our teaching hospitals is obsolete. That position should be a CEO and open to competition from anywhere. These hospitals should no longer be exclusively funded by government but be encouraged to commercialize themselves. It really is a disgrace that with everything on ground in LUTH that institution does not make money. Another Snake swallowing JAMB story That place could make more money than some state government annual budgets. That will happen if it comes under an entrepreneurial management team The first thing to be done is to collect data on those who travel abroad for healthcare, their diagnosis and budget and aim to get a share of that. No one teaches doctors to think like that They are trained to spend without thinking of where the money comes from. |
