You cannot proceed because you've not uploaded your medical fitness certificate..it's very important... Besides your list is too long.

When you're done with your application you'll be told to print out your referee and reference page.

The referee form will be printed out and filled manually, you don't have to scan and upload it. You have to take it along with the other documents you uploaded when you're shortlisted for screening. You're not to fill it online.....
And yes you need a copy of ur referee's identity card. All you need do is complete and submit your application before the next page will be displayed(that's where you'll be asked to print out ur referee and reference form)The instructions are on the referee form.

someone pls help..I went to print NPS today but discovered d height I put was 1.68 instead of 1.72 and passport doesn't show on d reference page...should I reapply or just leave it like dat pls..since no way to correct it on dis current application

Really? Well I guess he has to delete everything and upload in the order it was listed and he also has to include his medical fitness certificate....but the list is quite long though.
