Alright. First you didn't let us know the what the cough is like. Whether you cough out anything and I'd you do what's the nature of what you cough out.

You didn't let us know the kind of drugs you have been on because drugs are specific to the kind of cough.

Are there other symptoms that you feel? Any night fevers for instance and do you think you have lost weight?

Have you tried to do chest xrays and TB testing?

Are you hypertensive and on some kind of hypertensive drugs? Some hypertensive drugs can cause you to cough.

It could also be related to allergies. That's why your doctors also must review your medication history.

Follow the advice of pacificman, georgeceekay or lexusgs430. In addition get a second or third opinion from a respiratory physician or cardiologist.
If you have been coughing for four years and you are not yet dead, you are unlikely to have tuberculosis, cancer or heart disease. There is hope. Environmental allergies, drug adverse effects , post nasal drip or mild COPD are my first thoughts.

And you are aware that swelling in the area of the lymph nodes is a common symptom of cancer that starts in the lymphatic system, such as non-Hodgkin lymphoma and Hodgkin lymphoma.

Have you done gene expert? There are some tuberclosis that are resistance to most refampicin or have you done fnab( fine needle aspiration biopsy)? To accertain if isnt a pulmonary tuberclosis? Though the procedure is tedious and capital intensive. But gene expert is free in most teach hospital

This could be an allergic reaction to even some drugs that you might be using for treatment of another ailment. My mum coughed for close to 3 years, we went to Me Cure at Oshodi, we went for tuberculosis tests and HIV Hepatitis B, you name it. Until she went to one pharmacy shop and the in-house pharmacists just asked her if she had any other condition that she is treating at the moment, and she replied that she has been taking drugs for her heart condition. she mentioned all the drugs that she has been taking all the 3 years and he just spotted out one of them and the reason for her cough. as soon as she went back to the hospital and complained they gave a substitute for that drug and that was the END of the 3 years curse of cough.

What tests have you had done?
If it worsens at night especially if you lie down you might need to rule of the following
* postnasal drip.
* asthma, especially cough-variant asthma, which causes a cough as the main symptom especially at night
* acid reflux or gastroesophageal reflux disease (GERD)
