To answer your question:
For any stock, you'll be qualified to receive dividend in as much as you buy it on/before the qualification date.
After the qualification date, the share price of such stock will be marked down and anyone who buy after 'mark down' will not receive dividend.
All you need to do if you want dividend is to have such stock in your portfolio on/before the qualification date and, register for e-dividend payment by filling the e-dividend form and submit thru your bank or the registrar I think. Dividend will be paid to your acct on 'Payment Date'.
E-dividend registration for any stock is ONCE. You keep getting your dividend every year ......provided you have such stock in your portfolio "when" and "if" dividend is declared by such coy in a particular year in view.

Yes. Bank acct with your broker preferably, bearing same name with the name you used in registering/opening acct with your broker (I have my reason for saying it this way).
Download the registrars' forms (if not yet done), fill it and take it to your bank.
