The idea of the first coup in 1966 was originated by a Yoruba Major named Ademola Ademoyega. Their plan was to make Awolowo president. But the coup failed. That coup was foiled by two Igbo men named General Aguiyi Ironsi and Lt. Col Chukwuemeka Ojukwu.
At the time Ironsi took over, he maintained the regional governments. Lt. Col. Chukwuemeka Odumegwu-Ojukwu was appointed Military Governor of Eastern Region. Others were: Lt.-Cols Hassan Usman Katsina (North), Francis Adekunle Fajuyi (West) and David Akpode Ejoor (Mid West).

It was Gowon who broke the regions into states. Ironsi was already dead by the time the states were created in 1967.
In a move to check the influence of Ojukwu's government in the East, Gowon announced on 5 May 1967 the division of the 3 Nigerian regions into 12 states: North-Western State, North-Eastern state, Kano State, North-Central State, Benue-Plateau State, Kwara State, Western State, Lagos State, Mid-Western State, and, from Ojukwu's Eastern Region, a Rivers State, a South-Eastern State, and an East-Central State.
Don't forget that even at Aburi in Ghana, Ojukwu insisted on regional governments.
