you don't get the point nii, referee form is different from the two you are saying. First page that d court will sign is an attestation of the parent and d second one is for ur local government or any senior officer to attest to the fact that u are an indigene of the state. But your reference form is to attest to your character, age and all other things by any of the officers who are eligible to sign the attestation of form. They are not the same thing

CDS is chief of defense staff..... Army, Navy and airforce are all under defense..... The CDS is a general.... Four star... He is the only one to wear that rank.... Ordinarily Lt gen and it's equivalent are only for the chief of Army staff, naval staff and Air staff except this time Buhari gave Lt gen adeosun....

Traditionally, the CDS is the most senior. If not he can't summon any of the service chiefs as junior to any of them.
Buratai is the most senior in the army, but we now have two other Lt.Gens Adeosun and another who is now in charge of UN missions.
