History taking involved a lot of things; family, social, medical, surgical, nutritional, obstetrics/gyaeny, immunization, allergies etc. So, the doctor is not wrong.
As I say earlier, head to toe also encompasses system by system.
Alimentary system, musckloskeletal,reproductive/urinary system, nervous system etc. The reason for been to the hospital is to have a holistic treatment. You may present with the Hx of symptoms of malaria and further probes may established the existence of UTI(urinary track infection).
Dr need to treat both.

I assure you, all those questions are IMPORTANT. When clerking a patient (especially females of reproductive age), a good gynaecologic history is very, very important...irrespective of what the presenting complaint is.

As a matter of fact, a doctor could be queried by his superior if he fails to ask questions like sex history while clerking...

A cardiologist not asking you doesn't mean it is a wrong question that cannot be asked of patients. I can tell you a 100 possible questions a cardio can ask you but you were certainly not asked all of them. My point in all of this is if you don't understand something you ask. As for the relevance I'd have to paint a clinical scenario for you to get the whole picture and I don't have that time or patience to type but just know that for a woman or man who's major physical activity happens to be sex the cardio can ask about it as a measure of decline in cardiopulmonary function. It's like asking a patient how long he/she walks b4 he or she gets tired and then comparing it to his pre morbid state. Hope this sheds a little light.
