This is the Islamic position

The Prophet (blessings and peace of Allah be upon him): “The Pen has been lifted from three: from the sleeper until he awakens, from the child until he reaches puberty and from the insane person until he comes to his senses -- or until he comes round.” Narrated by Abu Dawood (4403)

So for the period of mental illness: No record is kept for such a person

Wise and tough question, the things we have no answer to, are answered by God! He should ve a way of judging them.

I think he will be judged based on before his madness started - when he was in the right frame of mind.

Also, some lunatics are actually demon-possessed, so how does God judge someone who is possessed?

Are they born-again? If No, then God's judgment since the time of Adam - Genesis 3 vs 16 - 24 - is what stands. undecided

Who is Abu Dawood that we should care about his narration... what's the source of the narration

All have sinned and fallen short of God's glory {Romans 3:23} so through Adam death rule as King over mankind {Romans 5:12,17} whether a person proved to be good or evil he or she must grow old, get sick and die! Psalms 49:7-9

Resurrection is the only hope of seeing our dead loved ones again because God promised to raise them back to life in perfect health! Job 14:13-15

If a person doesn't have anything worthwhile to be remembered for such a person will never be resurrected they're gone for good! Psalms 9:17

But if there are God's servants who will be happy to see such a person brought back to life in Paradise God will raise the dead so as to bring joy to the hearts of His loyal servants in Paradise. So if a mad man had developed a good relationship with God's people before his madness begins it's certain that God will remember him for the sake of His loyal servants whose tears will turn to joy at the sight of their beloved brother in faith who died as a madman! Revelations 21:3-5

God bless you and may you have PEACE! smiley

Madness is one of the odyssey of Soul. It teaches us to cooperate with the laws of God. We either obey the divine law or suffer the consequences. Reincarnation eventually brings us back to face our actions (karma).

God is the only wise and righteous judge. He has different strokes for different folks.
Leave Him with His judgment.
Work your own salvation with fear and trembling.

No, It's brain malfunctioning!
Just as people loose the sense of sight, smell, taste, sound and touch the same applies to the malfunctioning of the brain. It could occur accidentally or through man's blunders.
Demons are highly intelligent powerful beings, they know what to do that will affect man's senses of either taste, sight, touch, smell and sound. When they attack these five senses a human may appear to be walking with his head upside down to the affected patient and his mouth will surely speak of what he's seeing hence others will say he or she is mad! undecided
