Your license no is on the temporary DL. You need it and your Date of birth to apply for reissue. You also need a police report and court affidavit.

You can choose to go to a licensing office or do it yourself.

The procedure is fairly easy.

Note, I think step 2 just for New DL.

I can't help, sorry.

At the same price? Can't say, try the BIR office or any Licensing center near you. I am sure you won't spend more than 8k for renewal if you go there Knowing the break down costs.

The official rate is 6000 for 3yrs or 10,000 for 5yrs. Either one has a 350 bank charge. They may likely charge you 500 for form, 500 for internet, 200 for lamination of temporary DL or another I may not be aware of in your location.
