I can't recall that, but the Guardian form is meant to be signed by your parents or Guardian and a court as a witness to ur parents attestation should in case anything happens during training or screening ur parents or guardian will never hold NA for it. Thanks

Truth is I never expected to be among those shortlisted today(ssc) but it did happen.. now, I'm faced with a major challenge.
*I don't have the original degree certificate but I have the statement of result

*With reference to my SSCE, I'm using my recent NECO results which I collected in 2019. No original yet and I graduated from Uni in 2015

*My Date of birth with bank and NIMC is in total nagation with birth cert though I have declaration of age

Please with all this compounded issues, is it advisable I make this journey..?? I no get person oh, only God!

Although the year is correct and I have an affidavit... Please, is it a NO NO for me so I don't kuku stress myself to be traveling to Kano or I should use WAEC statement of results gotten from my Sec Sch? Thanks

I had similar issue back then, we were allow to go and do court affidavit in kaduna. After I submitted the affidavit, I was cleared. You said u have ur own court affidavit then you are gud to go. All the best.

Filling it that way is still very much better than going to the Screening venue with yet-to-be filled Reference form.. so you will be fine

(1) Yes Nah.. The reference forms are expected to be signed by 2 different people
(2) No problem with that. Just let them realise that its ur school that's yet to give U guys the Original cert

GENERAL INSTRUCTIONS FOR SHORTLISTED CANDIDATES OF SSC 47 AND DSSC 26 – 2021
GENERAL INSTRUCTIONS TO CANDIDATES

1. The Selection Board for SSC 47/2021 and DSSC 26/2021 will take place from 16 January to 5 February 2021 and 1 – 14 February 2021 respectively at Falgore Training Area in Kano State. Shortlisted SSC Candidates are to report to the venue on 16 January 2021 while Shortlisted DSSC Candidates are to report on 1 February 2021 all with the following:

a. Writing materials including ruler and eraser.
b. 2 pairs of plain white short sleeve vests.
c. 2 pairs of navy blue shorts.
d. Canvas shoes/trainers and white socks.
e. Toiletries.
f. Set of cutleries including plates and drinking cups.
g. Beddings to include, blankets, bedspreads and pillow cases.
h. 4 copies of full size colour photographs at standing position in suit.

2. The Selection Board will involve physical, medical and academic tests. There will also be an oral interview for all candidates.

3. Candidates will be required to produce original and photocopies of the following:

a. All academic/professional certificates including degree/HND, WASC/GCE or equivalent, testimonials, NYSC Discharge or valid Exemption Certificate as applicable in a file jacket.
b. Valid birth certificate as endorsed by the National Population Commission, hospital, Local Government Council of birth or valid age declaration.
c. Valid certificate of state of origin.
d. Military Identity Card (for Service personnel).
e. Letter of recommendation by the commanding officer for military personnel.
f. Letter of sponsorship in tertiary academic institution for military personnel.
g. Printed Bank Verification Number (BVN) certificate authenticated by a bank.
h. Completed copies of the print outs from the portal including referee forms.

4. Candidates are to note that any alteration on their documents will render the documents invalid and eventually lead to disqualification.

5. Candidates will be fed and accommodated for the duration of the interview.

6. Candidates would be responsible for their transportation to and from the interview venue.

7. The use of personal/private vehicles will not be allowed throughout the duration of the interview.

8. Visitors will not be allowed throughout the duration of the interview.

9. Shortlisted SSC Candidates will report at Nigerian Army Training Area in Falgore Kano State on 16 January 2021 from 8 am. Candidates who arrive later than 9am on 17 January 2021 will be disqualified.
10. Shortlisted DSSC Candidates will report at Nigerian Army Training Area in Falgore Kano State on 1 February 2021 from 8 am. Candidates who arrive later than 9am on 2 February 2021 will be disqualified.

10 A high standard of discipline is expected from all candidates. Candidates who violate any instructions during the selection board would be disqualified.

1. The reference form and LGA form can be signed by 2diff ppl, but make sure that the LGA form shud be signed by LGA chairman or his secretary on behalf of the chairman.

2. Statement of results is allowed, but u most come with scratch card for confirmation.
