my sister, I havnt heard of that b4.
Maybe when u get to camp. U fill it before collecting ur kit, probably.

Yes there is a form that is filled as regards your shoe, cloth size etc...I don't know about other schools, we filled ours in school....I doubt if it is filled in camp...Besides all those things na formalities, @ camp they give the kits @ random, if what you are given is not your size (which will never be), you look for someone to exchange with, better still you shape it to your size and taste.
