Fertility tests do not guarantee conception.

However, being the only son is not a tenable reason. Perhaps age is not on his side (40+) and he is eager to have a child to bear his name. He might even be specific for a male child. Perhaps also the girl has a sordid past, and he is sceptical about her ability to conceive.

Anyway, the choice is hers to make. Some people get married for love, others for children.
