Misolex:I think so because you need it before you will be cleared and you can't print your acceptance payment receipt if you haven't thumbprint, it's part of document to bring along on clearance day |

For those that are yet to do their Medical clearance I decided to make this write-up for yOu guys.. I hope this help [b]How to go about yOur Medical clearance Make sure yOu come with ur original receipt of Medical payment #2,200 4 photocopies of tHe receipt A̶̲̥̅̊η∂ ur Original Medical form tHe one you filled with biro A̶̲̥̅̊η∂ a photocopy of it.. Just the. Original A̶̲̥̅̊η∂ one photocopy of it A̶̲̥̅̊η∂ 2 recent passport photographs You will have to get there as early as possible.. After that you will put down ur name on tHe register.. Then yOu wait till they start calling names A̶̲̥̅̊η∂ giving Tally.. After dat you will stay on tHe queue till it gets to ur turn after that you will Pay 300 for Diary.. After tHat they will measure ur height A̶̲̥̅̊η∂ take ur weigHt.. Then you will be given a green Medical folder that contains some form A̶̲̥̅̊η∂ a yellow card in it, fill every necessary content of tHe Green foldrer A̶̲̥̅̊η∂ the forms in it.. After tHat head straigHt to tHe X-ray centre A̶̲̥̅̊η∂ submit one of the photocopy of tHe Medical fee receipt then wait till tHey call you to come in for ur X-ray don't be afraid uHn.. All you need is to pull of ur shirt A̶̲̥̅̊η∂ singlet(guys) for females you will pull of ur sHirt A̶̲̥̅̊η∂ bracelets' as well they sHould give yOu a nylon stuff to put on.. Å̶̷̩̥͡•̸Ϟ not tHat sure tHo.. After tHat continue with tHe queue for eye A̶̲̥̅̊η∂ blood pressure' test.. Then go back for ur X-ray result..The next day try to go there as early as possible for Urine test.. Coz urine test is done in tHe morning.. Then you will be qiven a small test tube after that navigate ur way to tHe tOilet den discHarge ur urine inside tHe tube.. After tHat you will drOp a slip from tHe green Medical card that's where they will record the result of ur urine test then go back for tHe result in tHe afternoon.. After all the necessary test has been conducted A̶̲̥̅̊η∂ you have all your results ready with you, head to one office where they will help you bind your file.. After that you will then need to see the doctor he will then stamp ur files/document A̶̲̥̅̊η∂ ur yellow card(the stamp reads the polytechnic Ibadan Medical officer) after that you will need to go fOr another stamp where they binded the files.. Abeg help me out oooo one of my friend dat we wrote ibadan poly post utme together call me this morning dat he was offer admission and I ask him how he now later told me dat he went to ibadan to go and check the admission list dat he found his name bt wen we log in inside his account there was nothing like u av been given admission ther abeg we really happen I don't knw oooooo ?? |
