Its true that saying 'm in MBBS' is grammatically wrong. I once posted it here that I am not here for lexis and structure.. I am here to communicate. and so far d person I replied and any other prsn understands what I meant, m satisfied ..
u shld knw saying MBBS is just like giving a short and understandable form..

I cant say it in an official gathering.. but if I say it hear, no one cares...its not like I get coins.

for next tym.. it is true that u take clinical courses.. but nobody specializes in 500level as ur post suggested..
In your clinics, u take courses on medicine,surgery, O & G n d likes.. but all students take d same courses.
the specialization comes after graduation and after you must have passed it primaries (an exam u take after ur nysc).
What we r saying is... once u have an MBBS... u r a graduate of medicine and surgery.. not a graduate of Opthalmology or gynecology or d likes.

mbbs is just a general certificate.. it doesn't make u specialized in any medical or surgical specialty/subspecialty..
