Search around here for tutorials. There are many of them. Just learn as much as you can. Even those menu generators, you can start coding up yours after some tutorials here.

Architecture.Before you design a house you must know the perfect design or structure.So,the Structure is my greatest troubling.I dont know how to determine different design used for different website.

Do you know CSS? It will really help you make perfect layouts for your pages be they 2 or 3 column or whatever. Ask yawa-ti-de for her ebook on CSS, visit www.dynamicdrive.com and study their layout tutorial. Google houses many tutorials on this too. Research.
