How do guys actually know the perfect size of their ladies engagement rings especially when the lady doesn't wear a fancy ring the guy could stylishly pick as a sample to know her actual size?

Don't you guys think the guy would have asked one way or the other to know the size to get which in turn makes the lady in question expectant?

So I wonder why ladies in most cases act surprised.

Best way would be to take a ring of hers to your local jeweller or talk to her sister if she has one or friend. Someone usually has to help the man out.
The friend can pretend they are trying on rings etc or playing around with jewellery and she'll get a good idea of what fits.
Then buy from a reputable jeweller who would resize for you if you guessed wrong. You'll have to pay for the service though.
