You don't get it.

A surgical operation was done on the baby when the pregnancy was about 5 months to remove a tumor. The baby was thereafter returned to the mother's womb who carried the pregnancy to 9 months after which she was delivered by another operation. Thus the baby was technically born twice.

Cass is the main surgeon,the Nigerian doctor is his co surgeon or assistant,but be sure in the next few years,the Nigerian wud av taken over from the Dr Cass after he retires or the Nigerian doc gets recruited to head another facility where he wud be performing these rare surgeries on his own,having honed his skills.
