please doctor help me, three days ago I experienced a severe pain and inflation on one of my testicles though right now the pain has reduced unlike days back as for the inflation can't really tell if it is decreasing now although it not growing bigger, please I need ur advice and help urgently on what to do, |

nne_mak:It sounds like you have chronic sinusitis, which is the inflammation of the sinuses. If it's chronic it means the place is filled with long standing fluid and sometimes pus, causing pressure on nearby structures like the eye, nerves, and muscles. If you have fever, or headache, blurry vision, or dizziness; then you have to emergently go see a doctor. Eventually, you would need an ENT doctor to surgically clean out these sinuses, meanwhile try to identify and avoid aggrevating factors in your environment, such as dust, pet dander, plants, termite, etc. Also you can try nasal decongestants----like neosynephrine drops if it wouldn't raise your blood pressure. An old, but effective method (used by grandmothers all over Nigeria) is the steam bath-- where you're exposed to rising steam, you breath this steam for a few minutes, usually in an enclosure, then the mist simply opens up your sinuses. |

Charles A.: The best advice I can give anybody with testicular pain is to go see a doctor ASAP. Even if it feels better, you still have to watchful for re-occurrance. |

Hi doctor, I feel some faint sharp pains in the middle area of my chest a bit to the left though, and I also feel some kind of sharp pains again at the back of my neck at the left side. What can this be, I have been this for more tham a year now, done my BP it was OK, I did a lipids test it was 181 the doctor said it was fine. I did an ECG again the doctor said nothing but I should go and do a stress test and a heart Scan. I really need to know what might be happening because I do not think my doctor is communicating enough. Though I an very healthy apart from these faint pains I feel often. and I must say I sleep well too. |

@Naijaking1 Thank u so much for your concern.I have being treating staph for the past 2yrs as a result of staph cultured in the result in more than 3 lab centres. I changed to a new lab centre and heavy growth of coliform organisms was cultured in the semen.I went to my doctor he gave ciprofloxacin as a result of the lab result. I repeated the SFA after 2wk and 1wk interval and thus is the result; volume=5ml viscosity=normal color= grayish white motility; active=nil inactive25% dead=75% mophology; normal=40% abnormal=60% Count=12m pus 2-3hpf, epith nil,others nil heavy growth of coliform specie sensitivity; ciprofloxcin+++ pefloxacin+++ floxium+++ ofloxacin+++ My Dr gave me cipro for 2 wks and to repeat the text after a wk interval. Am almost true with the cipro |

Kevostica: I am surprised you had to take medication for staph for 2 years! First of all, I don't think you were treating staph, because the organism is not classically treated with cipro. However, it illustrates what I said earlier about understanding the microbiology and pathology of staph. Coliforms is a term used to describe a group of gram negative bacteria, it includes, E. coli/Klebsiella, etc. These coliforms are usually found in the the stool. I want to talk about coliform and staph, in this way, because, it indicates the likelihood of contamination either from collecting the specimen, or in the lab. Your seminal fluid analysis SFA is not good unless it was properly collected, stored, transported to the lab, and analysed. If properly collected, it could have shown the type of bacteria lining the urethral, spermatic duct, and so on. Any small contamination, could have yielded coliform and staph. If the specimen was not stored at body temperature, transported to the lab within 10-15 minutes, and analysed well, the result you show above will be nullified, and of no use. When I prescribe SFA, I usually demand that the lab state time of production, time of analysis, and temperature. If the sample is not analysed on time the sperm will loose motility, and quickly begins to disintergrate. So, without these information, it hard to draw any conclusion from the result. There're other organisms that could cause symtoms which can't be isolated from the lab using routine methods, like they use in Nigeria. When you cannot isolate chlamydia, trichominas, or ureaplasticum, or neisseria gonorrheae, which are the real pathogens, then contaminats like staph or coliform are reported. Cipro and similar groups of antibiotics are used against some of these pathogens. Finally, I would recommend that you take an antibiotic break, watch what happens, use protection during sex, and let you body heal. You can't take Cipro forever. |

Naijaking1 Thank you so much for your reply.I just resumed from the break after all staph staph here and there. At a time i was tired of the whole thing and decided to wait for sometime.Then before the 4month break d result count ;abt 25m with 40% active,moderate growth of staph.Then the time interval was like an hr (before examination). though there were usually breaks after 4-5 doses. After 4months of not taking anything i repeated and got heavy growth of coliform instead.count was abt 2m pus cell 4-6 active1%,dead and inactive 2% each. D Dr gave me cipro for 2wks.That was the test befor the one that was posted. So as it stands, do u still suggest i should gon on break again? Erection is somehow poor |

Kevostica: The staph or coliform most likely has nothing to do with your poor erection. Poor erection is treated in another department--endocrine urology. You have been at the microbiology department all this while. If poor erection is your main complaint, then you need to start all over; tell us your age, medical problems, medications you're taking, injuries/surgeries, etc. whether your poor erection is there all the time, sometimes, only with a certain sex partner, or every partner. Do you mastubate, do you have erection at night while sleeping? Forget staph/coliform and that unreliable post-1hour seminal fluid analysis for now. |

shezy: If you invite one man to have dinner with you, and you set the table for only one man, then the whole 10 million men in Lagos show up at your door, each trying to be the first to get in an enjoy the dinner with you, you would agree that the remaining 9,999,999++ men would be forced to stay outside, spilling into the street, and even blocking traffic well, well. That's the situation with sperms. All you need is just one, then the remaining other billions would have to be wasted. Keeping the penis inside the vagina after ejaculation will not force sperm to go do their job, and it wouldn't stop excessive sperm from spilling out either. So, there's nothing wrong with spilling sperm after sex, because the one single sperm needed to do the work of fertilization has already probably gone inside. If you want to get pregnant as it appears you do, please note that there're many other reasons on the woman's side why fertilization might not have taken place---such as periodic cycle, anatomy, medical condition, but spilling sperm out is not really on of them. 1 Like |

Naijaking1, My bro-in-law has bronchiolitis. After years of diagnosis and steroids, still no cure. Somedays, he would nearly pass away from lack of oxygen and would have to be rushed to A&E. The fam is very worried. Pls tell me there is hope for the guy. Secondly, I have rhinitis and should be going for corrective surgery later this year. Is this one of the reasons why my resting heart rate is 87 beats per minute or is it because I may be unfit? |

Hi Naijaking, Pls i av been diagnosed of ashtma for over 10 years but then in one year i might have attack only once especially during rainy season. Now since 2007 it's been always, i mean every week. I was given ventolin (both tablet and inhaler) inhaler worsen my situation when used but av been given predisnole, ventolin and antibiotic but no improvement, this health challenge has been a barrier to my purpose. I was advised to look around what could be my allergy, i noticed my office environment is dusty, what can i do to sustain my health. thanks |

Ibime: Brochiolitis is an airway disease caused by respiratory viruses, affects children less than 2-3years of age, and if proper diagnosis and treatment are applied has very small mortality rate. In our typical Nigerian environment, I would recommend an intensive educational campaign on the child's care givers, because the disease is contagious. It could be passed from patients to patients in latent forms--- I mean, parents could pick up the disease(since they're likely to be already immuned, wouldn't show any symptoms; but could 'give it back to the child' after a while), so you may have a circle of back and forth. Another source of the virus is certainly from the environment---furniture, animals, formite, secretions, airdroplets, etc. Treatment begins with symptomatic relief-oxygen, chest therapy, nebulizer inhalers, and anti-viral like ribavirin for persistent diseases. Antibiotics are not usually used, because its a viral infection, however when complicated with bacteria pneumonia, then antibiotics are applicable. Usually, if the mother pays close attention to the onset of every episode, she could easily identify the enviromental trigger--new soap, perfume, animal dander, foods, etc that accentuates the effects of the virus. The patient would most likely 'grow out of it', but there maybe late sequalea----pneumonia, chronic obstructive pulmonary disease, etc. Steroids like solumedrol are sometimes used for bronchiolitis, but the side effects should be made clear to the patients.--- high blood suger, high blood pressure, etc. Your pulse rate of 87 is perfect. No need to worry about that. I wish you luck with your surgery, I hope you have undergone medical therapy before seeking surgery. |

Opemiposi: Asthma is usually a reactive airway disease. An since you have to work in a dusty office, maybe your boss would understand and place you in a section with less dust. Haven identified your trigger, I believe you're half cured, all you need to do is get away from it. |

I mean no sleep for 24 hours and then falls to sleep for 18 hours at a stretch, too weak to even wake up. Its quite disturbing, wen he fell sick a neurologist (am not sure if such will still be recomended again) was d first point of call so i dont know d what consultant to contact now. We consulted a general medicine when we left hospital who instructed via phone to stop all medications and which we had adhered to strictly, but the above trend had been evident for the past two weeks. (sorry i initially used my mobile to post and its limited u know) |

Aladunni: Unfortunately, when a patient suffers a stroke or cerebravascular accident, there is always a higher than normal chance of stroking again. My recommendation is follow the gen med, in addition to the neurologist(I hope he is a neurologist). You may also need a cardiologist, because most of these accidents are of cardiac origin. Eczema skin disease treatment depends on how serious it is. Simple moisturising cream with no perfume or additives are best. When the skin dries, it cracks and infection might get in. Also try and avoid scratching. You will introduce infection from your fingernails to scratched surface. Some alternative medicine people feel there is a trigger food/thing. I have no objections to that type of thinking. If you find certain things or food trigger your eczema then avoid it. Serious diseases might require antibiotic/steroid cream or tablets if very serious. |
