The major issues to be tackled are substandard material strength, soil test report with an approved structural drawings, construction procedure/ quality control. Greediness by contractors and even clients, in the name of cutting cost needs to be checked.

More importantly, contractors should also pay more attention on the safety of their workers. Provide necessary PPE. Human are important than the cement and reinforcement. It is sheer excessive wickedness to spend much to secure your materials and not secure the manpower.
